Attending physicians on this service have significant clinical and research interests in the area of hemostasis and thrombosis and are available on a 24-hour basis. the close integration of the MUSC Coagulation Consult Service with the Clinical Coagulation Laboratory allows us to provide a number of unique services.

OVERVIEW

The Medical University of South Carolina has initiated a multidisciplinary approach for the management of thrombosis and bleeding. Physicians, pharmacist, nurses and scientist collaborate to provide state-of-the-art care for patients suffering from bleeding and blood clotting problems. A comprehensive set of educational videos, podcast, conferences and written materials are available to assist clinicians and patients in learning about these complex disorders.


MISSION

The primary mission is to assist in the diagnosis, treatment, and prevention of bleeding or blood clotting problems that produce pain and suffering in our patients. We are committed to providing advanced care that can diagnose, treat and prevent these disorders that can be effectively managed in an academic medical center.

ADULT and PEDIATRIC CLINICAL PROGRAMS

The program offers advice regarding:

identification of the cause of recurrent thrombosis,
the duration and intensity of anticoagulation,
identification of the etiology of inherited or acquired bleeding disorder,
the prevention of blood clots,
the treatment of bleeding disorders,
recommendations on using newly-approved anticoagulants


The adult hematology outpatient clinic is located on the second floor of Hollings Cancer Center and is staffed by hematologists and nurses devoted to the diagnosis and treatment of bleeding and blood clotting problems.
The pediatric hematology clinic provides comprehensive care for patients with hemophilia and other congenital bleeding and blood clotting problems. The clinic has board certified pediatric hematologists and nursing support to assist families in the care for children with these disorders.

VASCULAR MEDICINE at MUSC

Thomas Todoran, MD, MSc, is the Director of Vascular Medicine in Department of Medicine with an emphasis on developing and expanding peripheral vascular services.  Dr. Todoran completed a residency in internal medicine at Duke University Hospital, followed by a fellowship in Cardiovascular Disease at Barnes-Jewish Hospital-Washington University, St. Louis, MO.  Dr. Todoran is board certified in internal medicine, cardiovascular disease, and interventional cardiology.  He will be developing collaborations with interventional radiology, vascular surgery and medicine to provide patients with state of the art treatment of vascular disease through an integrated vascular medicine program.


VASCULAR SURGERY CLINIC and LABORATORY

Vascular surgeons skilled in the diagnosis and treatment of both arterial and venous disease are available for consultation. The vascular lab utilizes venous duplex scanning to assist in the diagnosis of venous disease. The vascular laboratory provides a wide range of additional services including carotid duplex, arterial duplex, aortic duplex, renal artery duplex and additional state-of-the-art noninvasive techniques.


PHARMACY MANAGEMENT

The School of Pharmacy at MUSC offers inpatient consultative advice regarding proper dosing, monitoring and treatment for bleeding and blood clotting disorders in collaboration for the primary physician and consulting hematologist.


ANTICOAGULATION MANAGEMENT SERVICES

Experienced pharmacists specializing in anticoagulation management care for outpatients needing to have anticoagulation monitored and adjusted. Patients are referred to the clinic by physicians and can be cared for at several different location throughout Charleston area.

A program is also available to allow selected patients to self-monitor therapy at home in collaboration with clinician.


CLINICAL LABORATORY SERVICES

The clinical laboratory has the capacity to provide diagnostic testing to define the cause of bleeding and thrombosis. Assays utilizing plasma, platelets or patient DNA are available to provide state-of-the-art evaluation. The laboratory is also prepared to assist in the monitoring of anticoagulation therapy with specialized testing to measure level of heparin, low molecular weight heparin, and antiplatelet therapies.


INTERVENTIONAL RADIOLOGY

The interventional radiologists are experts in performing thrombolytic therapy for arterial and venous thrombosis. In addition the can provide protection from pulmonary emboli by placing removable inferior vena cava filters in patients that cannot receive full dose anticoagulation therapy.


TRANSFUSION SERVICE

The blood bank at MUSC can support the bleeding patient by providing the necessary plasma, cryoprecipitate, recombinant proteins, platelets and red blood cells that are needed to manage life threatening bleeding problems.


CARDIOLOGY and CARDIOVASCULAR SURGERY SERVICES

A wide variety of cardiovascular disorders can promote arterial thrombosis. The MUSC Cardiology and Cardiovascular Surgery can assist in identifying and treating these disorders using state of the art diagnostic testing, noninvasive and surgical techniques.
