To obtain patient records, physicians may mail or fax release forms to:

In the event of an emergency, please call the Health Information Services office at (843) 792-3881 to advise a staff member about your urgent request.

Your request will need to include the following:

• Patient's full name
• Date of birth
• Social security number
• Description of what you are requesting
• Date of treatment

Requests for information on a patient who was not admitted or treated through our resident clinics should be addressed to both the department where the patient was treated and the attending physician to whom the patient was referred.
